Traditional or Indemnity Medical Benefit options give you the freedom to choose any licensed physician or hospital for treatment; however, if you use non-contracting providers, your out-of-pocket costs will be higher because you may be responsible for charges over the allowable amount. Whether you are at home or away, you have access to any licensed physician or hospital – including specialists – for covered expenses with no referrals. The choice is always yours. You must meet an annual deductible before the plan begins to pay benefits. Certain expenses, such as a non-emergency hospital stay, require preauthorization by calling the toll-free number on your ID card. Failure to preauthorize may result in reduced benefits.
The period of time (anywhere from 0 to 365 days) before the insurance policy will begin paying benefits. Also known as Elimination period.
Also known as "Duration of benefits," these are the limitations placed on the benefits you can receive (e.g., up to $150,000, two years, 5 years, lifetime, etc.).
Home healthcare typically includes visits to your home by a licensed Home Health Care Provider during which skilled nursing care, physical, respiratory, occupational, dietary or speech therapy, or homemaker services are provided. The benefit is usually equal to 50% of the Long Term Care/Nursing Home Facility Monthly Benefit Amount in most cases.
With an inflation rider, your daily benefit increases by a fixed percentage each year for a specified period. The lifetime maximum also increases proportionately.
The insurance industry considers certain hobbies or avocations – such as scuba diving, motor vehicle racing, skydiving, mountain climbing, and any type of flying – to be of high risk. If you engage in any of these, your premiums will probably be higher than an individual who does not participate in these activities.
A period of time (anywhere from 30 to 365 days) before the insurance policy will begin paying benefits. This is sometimes known as the waiting period.
Number of employees applying for coverage
The general rule is that if an employer offers group health coverage to any full-time employees,the employer must offer coverage to all full-time employees (defined as those working 30 or more hours per week).As for part-time employees (defined as those working 20 to 29 hours per week), the employer has the option of whether to offer coverage to them. If the employer offers coverage to any part-time employees, all of them must be offered the coverage.
These rules apply regardless of the medical condition of the employees.In other words, any eligible employee can't be denied coverage based on previous medical problems, otherwise known as pre-existing conditions.
In addition, any dependents of eligible employees are also generally eligible for coverage under a group plan. Dependents include spouses, children, and in some cases, unmarried domestic partners. Dependents cannot enroll for coverage unless the employee has enrolled.Different eligibility requirements may exist for group life, group long/short term disability and group long-term care insurance. This includes unmarried children less than age 27 who depend wholly upon you for support and maintenance. Stepchildren, foster children, and legally adopted children may be included as your own children provided they depend upon you for support and maintenance. You may enroll your children without also enrolling your spouse but you must enroll in order to enroll your children. Children are eligible for Major Medical Insurance from birth.

Long-term Care insurance is available for your spouse, parents, parents-in-law, grandparents and grandparents-in-law may also purchase coverage. TMAIT can also provide coverage for your older adult children please consult a TMAIT representative for details.
A physician may enroll his/her eligible dependents for Dependent's Personal Accident Insurance however; a newborn child will not be covered for Personal Accident Insurance before the quarterly billing date following birth. You may enroll your children without also enrolling your spouse but you must enroll in order to enroll your children.
