With the waivers, a solo or small practice owner could effectively create a “group of one” and just enroll themselves and their family in a group PPO plan.
Your employees could also receive this coverage, but the practice will not be required to pay for their coverage. Even if none of the employees take the coverage, the practice owner can still enroll in the group PPO plan as a "group of one".
